Enrollment Requirements

1. Fill out the Pre-Registration Form Now by clicking Enrollment Form button below. 
2. PSA Photocopy
3. For transferees, Form 9 (Report Card) and Certificate of Good Moral Character from your previous school
4. For old students, have your clearance cleared and submit the school copy of the clearance to the Office.
